Welcome to Meditrek. By accessing or using the Meditrek App or Meditrek Access, you agree to be bound by these Terms and Conditions. If you do not agree, do not use Meditrek.

1. Definitions

• “Meditrek App”: Platform enabling patients to record treatments, medications, medical metrics, and upload medical documents.
• “Meditrek Access”: Platform enabling healthcare providers (HCPs) to access patient data shared via Meditrek.
• “User”: Any individual using Meditrek, including patients and HCPs.
• “HCP”: Licensed healthcare professional authorized to access patient data.
• “Data”: Any health, medical, or personal information uploaded, recorded, or generated in Meditrek.

2. Data Collection and Use

Collection of Data

By using Meditrek, you consent to the collection, storage, and processing of all data you enter, including medical history, medications, measurements, reminders, and uploaded documents.

Storage and Security

Data is stored on secure servers using industry-standard safeguards. Absolute security cannot be guaranteed, and all data is stored at your own risk.

Sharing Data

Users may choose to share data with HCPs. Meditrek does not access or use shared data beyond facilitating platform functionality.

Third-Party Services

Integrations with third-party services are governed by their own terms. Meditrek is not responsible for third-party actions or failures.

3. User Responsibility

• You are responsible for the accuracy and completeness of all data entered.
• Medication and treatment reminders are informational only.
• Meditrek is not liable for missed, incorrect, or ignored reminders.

4. Use of Meditrek Access (HCP)

• HCPs may access only data voluntarily shared by patients.
• HCPs are fully responsible for interpreting data and clinical decisions.
• HCPs must comply with applicable medical, privacy, and professional regulations.

5. Disclaimers and Limitation of Liability

• Meditrek is provided “as is” and “as available.”
• No warranties regarding accuracy, reliability, or fitness for purpose.
• No guarantee of adherence, outcomes, or improvements.
• No liability for technical failures, data loss, or downtime.
• No liability for indirect, incidental, or consequential damages.

6. Ownership and Intellectual Property

• All platform software and content are the exclusive property of Meditrek.
• You retain ownership of your data.
• You grant Meditrek a limited license to process data to provide services.
• User feedback may be used freely by Meditrek without compensation.

7. Indemnification

You agree to indemnify and hold harmless Meditrek from any claims, damages, or liabilities arising from your use of the platform, data misuse, or violation of these Terms.

8. Termination

You may delete your account at any time. Meditrek may suspend or terminate access for violations, misuse, or illegal activity.

9. Changes to Terms

Meditrek may modify these Terms at any time. Continued use constitutes acceptance of updated Terms.

10. Governing Law
